What is a personalised care plan?
A personalised care plan is built around you as an individual, including your story, preferences and care needs. It reflects your routines, your likes and dislikes, and what brings you comfort and joy, as well as your health needs.
How is my care plan created?
We start by getting to know you, then complete a thorough and compassionate assessment. From there we build a plan that supports your wellbeing physically, emotionally and socially.
Who is involved in my care plan?
You and your loved ones. We review and adjust your plan in collaboration with you and the people close to you.
Can my care plan change over time?
Yes. As your needs change, your care plan changes with them. We continually review and adjust it so your care stays supportive, respectful and responsive.
Can a care plan include more than one type of support?
